In consideration of receiving a Tattoo, I agree to the following: I hereby declare that I AM 18 YEARS OLD or older and of Legal age (and have provided valid proof of age) and am competent to sign this agreement. Furthermore, I understand that if I provide a false ID or sign this form that I am breaking the law (tattooing of minors act) and am culpable for defrauding Killer Bee Tattoos and its Staff. I affirm that I am not under the influence of alcohol or drugs, and I am voluntarily getting this tattoo without duress or coercion. I have been fully informed of all inherent risks associated with getting a tattoo, including; Infection, scarring, detection of Melanoma (along with other skin disorders) and allergic reactions to tattoo pigments, latex or nitrile gloves, soap, tape, or any other products used by the Artist. I acknowledge that the Tattoo will result in a permanent change to my appearance and that my skin may not be restored to its pre-tattooed condition. I understand that after a period of time, the colours and clarity of my tattoo will age/fade, either due to unprotected exposure to the sun or the naturally occurring dispersion of pigment under the skin. I AM NOT/DO NOT HAVE; Eczema, Psoriasis, transmittable diseases, immuno suppression, Diabetes, Epilepsy, Haemophilia, Heart Condition, Taking Blood-Thinning Medication, Pregnant/Nursing, HIV, Hepatitis A, B or C, Anxiety, Prone to Fainting, Mental impairment that may affect my judgement in getting a tattoo, Recipient of an organ or bone marrow transplant, Allergies.I affirm that both the Artist and Killer Bee Tattoos have given me specific instructions on how to care for the Tattooed area/Skin while it’s healing and I understand them. I acknowledge that it is unlikely, but possible, that the Tattooed Area/Skin can heal unsatisfactorily or become infected, particularly if I do not follow the aftercare instructions given to me. If any touch-up work is needed due to my own negligence, I agree that the work will be done at my own expense (an aftercare sheet will be provided to you following your appointment, aftercare details are also on our website). I have been shown and approved of the design prior to getting it permanently Tattooed, This includes the circumstance of a ‘free hand’ design (drawn directly onto the skin). Neither the Artist nor Killer Bee Tattoos is responsible for the spelling, misspelling, or meaning, of any word/language/character/image in my Tattoo. I entrust my Artist to use his or her discretion in the application of the design, based on their experience and expertise. The service I am purchasing is one of an artistic nature, therefore I have taken the time to view my chosen Artist’s portfolio of work that is provided in the waiting area. Both the style and technical application of the Tattoo artist provides is to my approval. I release all rights to any photographs taken of myself and my Tattoo and give consent in advance to their reproduction, in print or electronic form. I freely accept and expressly assume any and all risks that may arise from Tattooing. By Signing below, I verify all the above fields and acknowledge that I have been given an adequate opportunity to read and understand this document, which was not presented to me at the last minute, and I understand that I am signing a legal contract waiving certain rights to recover against the Artist and Killer Bee Tattoos. I agree to reimburse all Solicitors/Attorneys/Legal fees and costs incurred in any legal action I bring against either the Artist or Killer bee Tattoos. I WAIVE AND RELEASE to the fullest extent permitted by law, all Artists, Staff and Apprentices at Killer Bee Tattoos from all liability. Any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from the tattoo, whether caused by the negligence or fault of either the Artist or Killer Bee Tattoos. If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document
